JOB PURPOSE:  As a member of the Nathan Littauer Hospital Care Coordination team, the Patient Health Navigator is responsible for providing a full range of medical, social and case management services to high-risk patients in the ambulatory setting by providing an appropriate level of evaluation to identify patients with psychosocial needs, both related to illness as well as related to resolution of Social Determinants of Health (SDoH).  The Patient Health Navigator provides appropriate interventions for patients as well as assisting with patient/family access to necessary internal service lines and providers.

EDUCATION:  Possess at least a two (2) year higher education degree in Healthcare or Human Services.

PREFERRED: 

• Bachelor’s degree is preferred.
• Case Management experience is preferred.

LICENSURE/CERTIFICATION/REGISTRATION:  None.

EXPERIENCE:  Possess three (3) years’ experience in Healthcare or Human Services, with a least one (1) year in direct patient interaction.

SHIFT:  8:00 AM – 4:30 PM                  STATUS:  40 HOURS (full-time)
