Assistant Project Care Coordinator/Care Manager

Description:  The Assistant Project Coordinator/Care Manager is responsible to assist in all areas of revenue maximization not related to budgets or charges.  This will include, but not limited to; PCMH initial certification and recertification, MU certification for all EP Stages, organizing projects to achieve HMO and HEDIS incentives and collection of data to support Grant applications. 

Requires:  

  • Possess a high school diploma or equivalent.
  • Possess Associate’s degree.
  • Prefer Bachelor’s degree in Health Administration or related discipline.
  • Possess ICD9 – CPT4 coding knowledge.
  • Possess training and/or certification in Project Management.
  • Prefer process improvement, quality improvement, meaningful use and patient centered medical home recognition.
  • Possess a working knowledge of outpatient, third party reimbursement, operations of a primary/specialty care private practice, managed care and computer operations.  Possess knowledge necessary to provide care appropriate to the age of the patients served on their assigned unit.

Status:  8:30am – 5:00pm (40 hours)

From:  $17.30 to $25.95 hour.

Director of Social Work

Description:  The purpose of this position is to provide a Social Service Program that meets the psycho-social needs of the individual resident which will provide services, based upon a comprehensive assessment, which will assure the maximum attainable quality of life for the residents, the residents emotional and physical we–being, self-determination, self-respect and dignity, taking into account the individual resident’s special physical or age-related needs.  The population served includes adult, geriatric residents.  Under the supervision of the Administrator, the Director of Social Work is responsible for the supervision of social work services within the facility. 

  • Possess a high school diploma or equivalent.
  • Possess an Associate’s of Arts Degree in Humanities & Social Services.
  • Prefer Baccalaureate Degree from an accredited college or University in Social Work or Gerontology.  Holds a Master’s Degree in social work or is a Certified Social Worker and has pertinent experience in a healthcare setting; experience with geriatric population preferred.
  • Possess a New York State Drivers License.  Certified NYS Screener or obtains certificate within the first month of employment.
  • Possess experience in long-term care and/or healthcare.
  • Prefer experience in geriatric long-term care.

Status:  8:00am – 4:30pm (40 hours)  Hours will fluctuate depending on departmental needs.
