Social WorkerDescription: The purpose of this position is to provide a social service program that meets the psychosocial needs of the individual resident which will provide services, based upon a comprehensive assessment, which will assure the maximum attainable quality of life for the residents, the residents emotional and physical well being, self determination, self respect and dignity taking into account the individual resident’s special physical or age-related needs. The population served includes adult, geriatric residents.Possess an Associate’s of Arts degree in Humanities and Social Services.Prefer Baccalaureate degree from an accredited college or university in social work or gerontology. Holds a Master’s degree in social work or is a Certified Social Worker and has pertinent experience in a health care setting.Possess a NYS drivers license. Certified NYS Screener or obtains certificate within the first two months of employment.Possess experience in long term care and/or healthcare.Prefer experience in geriatric long term care. Experience in areas such as Trauma Informed Care and Substance Abuse Disorder treatment preferred. Experience with discharge planning process preferred.Status: 8:00am – 4:30pm (40 hours)From: $25.14 to $36.45 per hour.
