Description: The MDS Coordinator Nurse is a LPN or RN who works under the direction of the Director of Nursing.  Responsible for the input of information into the MDS, coordination of all MDS assessments, CAAS and Care Plans under the supervision of the Consultant RN MDS Nurse.  Work is performed in accordance with established Nursing Home standards, Nursing department structure, established regulatory requirements and consistent with the philosophy, goals and objectives of the Nursing Facility and Nursing department.  The MDS coordinator will coordinate and lead care conferences.

  • Possess a high school diploma or equivalent.
  • Possess an AAS degree from an accredited school of nursing.
  • Prefer Bachelors of Science Degree in Nursing.  MDS Certification and RAC.
  • Possess current NYS license for Practical Nurse or Registered Professional Nurse; American Heart Association Basic Life Support (BLS) certification to be obtained within six (6) months of hire or transfer.
  • Two (2) years of Medical/Surgical or Long-Term Care experience.
  • Prefer two (2) years of Nursing Management experience in a Skilled Nursing facility.

Status:  8:00am – 4:30pm (40 hours) Hours will fluctuate depending on departmental needs.

Rate of Pay:  $27.71 – $ /hour
– Compensation for this position is based on individual skill set, experience, and qualifications.

LPN II – Emergency Care Center

***There is a $10,000 sign on bonus for full time LPN’s and a $5,00 sign on bonus for part time LPN’s***

Description:  The Licensed Practical Nurse II is a care provider who assumes responsibility and accountability for their patients within the limits of their education and the Nurse Practice Act taking into account the individual patient’s special physical or age-related needs.  The limits of the Licensed Practical Nurse’s responsibility as defined by hospital policy (see Generic Structure Standards) in keeping with the New York State Code, Rules and Regulations, and the NYS Education Department. 

Required:  

  • High school diploma or equivalent.  
  • Graduation from an accredited Licensed Practical Nurse program or completion of required components to satisfy LPN licensing examination.  
  • Valid NYS registration as a Licensed Practical Nurse.  
  • American Heart Association Basic Life Support (BLS) certification within six (6) months of hire or transfer.  
  • Completion of IV course & blood administration course within three (3) months of hire or transfer.  
  • Prefer one (1) or more years experience in an acute care hospital or previous experience in an acute healthcare setting. 

Status:  9:45am – 10:15pm (36 hours)

From:  $21.71 – $26.25 per hour.  

Licensed Practical Nurse (LPN) – Operating Room (OR)

DESCRIPTION:  The Licensed Practical Nurse is a care provide who assumes responsibility and accountability for his/her patients within the limits of his/her education and the Nurse Practice Act, taking into account the individual patient’s special physical or age-related needs. The limits of the Licensed Practical Nurse’s responsibility defined by hospital policy keeping with the New York State Code, Rules, and Regulations and the NYS Education Department.

Work is performed under the supervisor of a Registered Nurse in accordance with established hospital standards, Work Rules, Human Resources Protocol & Procedures, nursing department/unit structure, process and outcome standards, state regulatory and accreditation requirements, and consistent with the philosophy, goals and mission of the hospital and nursing department.  This takes into account the individual patient’s special physical or age relate needs.

DEPARTMENT: Operating Room (OR)

EDUCATION: 

  • Possess a high school diploma or equivalent education. 
  • Graduation from an accredited Licensed Practical Nurse program or completion of required components to satisfy LPN licensing examination. 
  • Employee is trained and prepared to care for the age-specific group served in their department.

LICENSURE/CERTIFICATION/REGISTRATION: 

  • Valid New York State registration as a Licensed Practical Nurse. 
  • American Heart Association Basic Life Support (BLS) certification within six (6) months of hire or transfer. 
  • Completion of IV course & blood administration course within three (3) months of hire or transfer.

SHIFT: 7:30 AM – 4:00 PM (40 hours)

RATE OF PAY: $24.72 – $29.26 / HOUR

Primary/Specialty Care Case Manager

The purpose of the Case Manager is to provide care management services for high-risk patients in the Primary/Specialty care population and work with behavioral health agencies outside the Nathan Littauer Hospital Primary/Specialty Care system to coordinate PCP services for the agency’s high-risk population.

EDUCATION: 

  • Possess a High School diploma or equivalent.  Graduate from an accredited Licensed Practical Nurse program. 

PREFERRED: 

  • Possess an Associates’ Degree with a focus on Care Management or related discipline
  • Possess Improvement; Quality Improvement and Patient Centered Medical Home recognition is highly preferred.

LICENSURE/CERTIFICATION/REGISTRATION:

  • Possess current NYS license for Licensed Practical Nurse.

EXPERIENCE: 

  • Possess prior Care Management experience.

SHIFT:  8:00 AM – 4:30 PM              STATUS: 40 HOURS

Licensed Practical Nurse (LPN) II (MED/SURG)

Description:  The Float Pool Licensed Practical Nurse II is a care provider who assumes responsibility and accountability for a group of patients for a designated time frame on an assigned unit within the limits of his/her education and the Nurse Practice Act taking into account the individual patient’s special physical or age-related needs.  The limits of the Licensed Practical Nurse II responsibility is defined by hospital policy in keeping with the New York State Code, Rules and Regulations and the NYS Education Department.

  • High school diploma or equivalent.
  • Graduation from an accredited Licensed Practical Nurse program or completion of required components to satisfy LPN licensing examination.
  • Employee is trained and prepared to care for the age-specific group served on their assigned unit.
  • Possess current NYS license for Licensed Practical Nurse.  American Heart Association Basic Life Support (BLS) certification within the first six (6) months of hire.  Completion of IV course, phlebotomy, blood administration course and EKG within six (6) months of hire.
  • One (1) or more years of previous experience in an acute hospital setting.

Status:   MED/SURG 3rd Floor Nursing/WEST – 6:45pm – 7:15am (24 hours)

Rate of Pay: $21.71 – $26.25 per hour.
