Assistant Project Care Coordinator/Care ManagerDescription: The Assistant Project Coordinator/Care Manager is responsible to assist in all areas of revenue maximization not related to budgets or charges. This will include, but not limited to; PCMH initial certification and recertification, MU certification for all EP Stages, organizing projects to achieve HMO and HEDIS incentives and collection of data to support Grant applications.
