Description:  Plans and coordinates activities related to transition planning.  Develops and implements a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care across the healthcare continuum.  Responsible for the collaboration with medical staff, patient, family, community agencies and/or facilities involved in patient’s care and the healthcare team by assessing, facilitating, planning and advocating for health needs on an individual basis resulting in quality cost-effective outcomes.  Acts as the Lead Social Worker and performs supervisory duties for the Social Work team in the absence of the Care Coordination Manager. 

Requires: 

• Possess a Baccalaureate degree or higher as a Social Worker or Registered Nurse.  Masters of Social Work preferred. 
• Valid New York State Registered Nurse license. 
• Possess five (5) years experience as a Registered Nurse Case Manager or Social Worker. 
• Possess one (1) year of supervisory experience. 

Status:  7:30am – 4:00pm (40 hours)  Position will require working one (1) Saturday per month. 

$27.71 to $43.30 per hour.
